Janani currently works in 10 districts of Madhya Pradesh (MP) in central India. Janani’s long-term plan is to cover 10 states in northern and eastern India (The other states are Chhattisgarh, Uttar Pradesh, Uttranchal, Rajasthan, Orissa, West Bengal and Assam, besides Bihar and Jharkhand)

With a population of 60 million and a high fertility rate of 3.1, the scattered population of Madhya Pradesh makes the challenge of 12% ‘unmet demand’ daunting.

Janani’s operations began in September 1998. In December 2000, a state called Chhattisgarh was carved out of the tribal areas of MP. Since then, Janani has focused only on the truncated state due to constraints in funding. Training centres, first in Bhopal and later in Gwalior, Indore, Satna and Jabalpur, were established. Till date, the programme has created 5,552 Titli Centres in the state.

Janani created 233 Super Titli Centres but restricted the coverage to the capital district of Bhopal and nine contiguous districts because of unavailability of donor supports. The social marketing operation continues in the rest of the state.

The clinic-cum-training centre in Bhopal has been operating since August 1999. From a narrow focus of family planning, the basket of services expanded in February 2002 to make available all reproductive health care services. The Surya Clinic protected 66,000 couples annually during 2006. Of these, 15 per cent were referred by the Titli Centres. The clinic became fully self-sustaining in June 2003. Over 60% of the revenues come from non-family planning, mainly gynaecological, obstetric, child care and diagnostic services. In June 2005, the Bhopal Surya Clinic was recognized by the government of Madhya Pradesh as a training center for training of MBBS doctor on safe abortion and Tubal ligation.

One more Janani owned clinic and one franchisee clinic is functional in the project districts. Sixty other doctors, mostly general practitioners, were trained in modern obstetric skills-- 22 of them currently use the Bhopal Surya Clinic for referral. The Bhopal Clinic has also trained 263 doctors in providing safe abortions with MVA technology.

In 2006, Janani in collaboration with the Government of Madhya Pradesh has delivered 250,000 CYPs through outreach sterilization services (OSS). In addition to this, Janani has been a major partner of the Government of Madhya Pradesh for implementing different health schemes e.g. Janani Sahyogi Yojanan, Janani Suraksha Yojana and Janani Bima Yojana.
