Dear Dr.:

We are pleased to provide you with a contract up to as a Consultant at our Patna Surya Clinic w.e.f as per the following terms and conditions:

1. Your service duration and payment will be as per the annexure enclosed and would be conveyed to you by us.

2. You will report to the undersigned or any other person assigned by him.

3. This contract can be extended further on mutual agreement.

4. A notice of minimum one-week is to be given by either party to terminate this contract.

5. Janani also reserves the right to terminate this contract without assigning any reason. In such case you will not be entitled to any payment/compensation/ reimbursement whatsoever for remaining period of the contract.

6. You will be required to return to the society all papers, documents, samples and literature which belong to the society and which may be in your possession at that time.

7. You will be required to keep all correspondence and other information or material given to you regarding our work as confidential and not divulge to any person or firm or company such information or correspondence.

8. If any dispute or difference arises during your service or any time thereafter the tribunals and courts in PATNA will have exclusive jurisdiction in the matter.

9. The work will be done in three shifts and you will be assigned any of the three shifts and you shall do the work accordingly.

10. You will hand over the charges to the appropriate person assigned for the next shift before leaving the premise.

11. During the course of this contract Janani would require to use your services in other parts of Bihar, Jharkhand or India. The decision for field level services lies exclusively and unilaterally with Janani and you shall be required to attend to the same within 48 hrs.

12. Regular meeting will be conducted in the clinic where you participation is essential.

This contract has been issued to you on the basis of information furnished by you about yourself. If any of the details and information furnished in your application form, or thereafter, are found to be incorrect or it is found to that you have concealed or withheld any essential detail, this contract is liable to be terminated forthwith on that score.
