Dear Mr. This has reference to our discussion with you. We are pleased to offer you a contract w.e.f. to conduct field inspection and reporting through 14 “ Monitoring Officers” in order to check the quality of functioning of the Regional Centres and Titli Centre network of Janani at the district, block and panchayat levels as per the guidelines laid down by Janani. You will be required to undertake the above inspection in Bihar and Jharkhand. Details of the inspection and reporting are given below: A Field Level: 1. Each Monitoring Officer will have to check whether the participant couples at the training campsite conform to the criteria provided by Janani, as per annexure 1. 2. Each Monitoring Officer will have to check the overall quality of the training at the campsite, as per annexure 2. 3. Each Monitoring Officer will have to do the back check of Area Inspectors' monitoring and sales work at the field level, as per annexure 3. 4. Other than the above mentioned services, the Monitoring Officers can be assigned any other fieldwork as per Janani's requirement. 5. Janani will provide the details for back checking of the Regional Centre and Area Inspector. 6. Janani will provide the pre- printed formats of the reports and supporting documents. B Reporting: 1. You will be required to consolidate all the reports of the Monitoring Officers on a weekly basis. 2. You will be required to submit
2 reports every week: 3. The consolidated reports (hard copy and floppy) of the previous week should reach Janani head office by Thursday, every week. If this is violated, an amount of Rs. 1000/- will be deducted for each late weekly report. C Payment: Details of the billing and payment structure are given below: 1. Payment structure for the Monitoring Officers will be as per annexure 4. 2. You will be paid 15% of the bill amount (other than bill against incentives and office expenses) as handling charge. 3. You will be required to raise
two bills which are as follows:- 4. Payment of all the bills will be released after six weeks of presentation of the bills along with the consolidated report. 5. Janani will conduct 25% back check on fieldwork done by the Monitoring Officers. In case any false report is detected, Janani reserves complete unilateral right to deduct 100% payment of that particular month of that particular Monitoring Officer. D Others: Other terms and conditions will be as follows: 1. Janani is in no way responsible for any dispute between you and the human resources employed by you. 2. Janani is in no way responsible for damage of any property owned by you. 3. This contract may be terminated by giving 15 days notice by either side. 4. This contract is valid till . 5. In case of any dispute, the matter will be resolved by arbitration under the Arbitration Act. 6. All disputes arising out of this contract shall be settled at Patna and only Patna Court shall have jurisdiction. Thanking you I hereby accept the terms and conditions as mentioned above.
