Dear Mr.

With reference to your quotation and our subsequent discussions we are pleased to offer you a contract for auditing the stocks at the 7 regional centers (Patna, Ranchi, Deoghar, Katihar, Gaya, Darbhanga & Muzaffarpur) w.e.f . The terms and conditions will be as follows:

1. You will be required to visit all the Regional Centres as per our instruction.

2. All your tour plan will be issued by Janani. You will be expected to begin the tour within one week of issuing of the tour plan. Each tour plan will consist of one visit per Regional Centre.

3. You will have to verify the stock register entries, supported by the relevant document produced by the regional centers.

4. You will have to verify the stocks physically available at the Regional Centres and the stock as per the stock register.

5. Janani will provide the list of items that have to be checked.

6. Janani will also provide the quantities that have been dispatched to the Regional Centres prior to your visit.

7. You will be paid Rs. 1000/- per visit per Regional Centre.

8. You will be paid a fixed amount of Rs. 450/- per day for food, local travel and other incidental charges.

9. You will be reimbursed up to a maximum of Rs. 600/- per day as lodging charges, on submission of original bills.

10. You will be reimbursed for travel (at actual) by 2nd AC or bus ex-Patna and between the Regional Centres on production of original tickets.

11. Telephone calls made to Janani from outside Patna will be reimbursed on submission of appropriate bills.

12. No other expenses will be reimbursed.

13. You will be required to submit separate report of each visit to the Regional centres within 3 days of completion of each tour plan as per the format provided by Janani. In case you fail to do so, Rs. 500/- will be deducted per visit per Regional Centre. Rs. 100/- extra will be deducted per day for every day of late reporting.

14. You will be required to raise one consolidated bill for each tour plan. Payment for the same will be made within 6 weeks from the date of submission of the above bill.

15. In case false reporting is detected, Janani reserves unilateral right to deduct 100% of the bill amount.

16. This contract is valid till .

17. This contract can be terminated with a notice of minimum of one week by either side.

18. In case of any dispute, the matter will be resolved by arbitration under the Arbitration Act.

19. For legal proceedings matters will be referred to the Patna courts only.

Please sign and return the duplicate copy of the contract as an acceptance of the job.
